PO-03-197 THE RISK STRATIFICATION VALUE OF LEFT VENTRICULAR EJECTION FRACTION AND LATE GADOLINIUM ENHANCEMENT ON CARDIAC MAGNETIC RESONANCE FOR PREDICTING VENTRICULAR TACHYARRHYTHMIAS IN NONISCHEMIC CARDIOMYOPATHY: A SYSTEMATIC REVIEW AND NETWORK META-ANALYSIS

The late gadolinium enhancement (LGE) on cardiac magnetic resonance (CMR) is associated with ventricular tachyarrhythmic substrate in patients nonischemic cardiomyopathy (NICM), while the left ejection fraction (LVEF) ≤ 35% still sole criterion for implantable cardioverter defibrillator (ICD) implantation. To perform meta-analysis to evaluate risk stratification value of LVEF and LGE CMR events NICM patients. PubMed Embase databases were searched studies reporting clinical outcomes (all-cause death, cardiovascular tachyarrhythmias[VTAs] major adverse events[MACEs]) based LVEF≤35% LVEF>35% or presence (LGE+) absence (LGE-), among NICM. Hazard ratios (HRs), both 95% confidence intervals (CIs), calculated using random effects models. Network was conducted outcome VTAs, four groups (“LVEF≤35%&LGE+”,“LVEF>35%&LGE+”, “LVEF≤35%&LGE-” “LVEF>35%&LGE-”) compared by HRs forest plot rank probability. Data from 7 (3002 patients, 1524 LVEF≤35%) included final analysis. Pooled demonstrated that increased risks all-cause mortality (HR 2.42, CI: 1.66 3.50), VTAs (HR: 1.67, 1.31 2.14) MACEs 2.36, 1.70 3.27). Meanwhile 51 (15553 6828 LGE+) pooled analyses showing those LGE+ more likely encounter death 2.08, 1.84 2.35) , 2.98, 1.97 4.50), 4.03, 3.12 5.20) 2.79, 2.36 3.31). analysis composed 3 (2342 patients) "LVEF>35%&LGE-" serving as control group, found a significant reduction VTAs-free survival "LVEF≤35%&LGE+" 5.2, 3.1 8.7) "LVEF>35%&LGE+" 5.8, 3.4 10.0), "LVEF≤35%&LGE-" showed no predicting VTA 1.9, 1.0 3.5). Both are highly sensitive predictor mortality, integrated significantly improved alone further discriminate who will benefit ICD.
